The purpose of these logs is to provide validation of clinical outcomes and supervision of professional development. Your logs should include the following two parts:

  1. Successful completion of CORE hours, with satisfactory explanation of each daily activity. Satisfactory midterm evaluation by mentor.
  2. A written narrative covering the following items:
    o Weekly reflection summary providing an overview of clinical or scholarly activities during the reporting time period (Weeks 6–10).
    o Commentary and evidence (examples) of progress toward achieving specific course objectives.
    o Reflections that include thoughts, feelings, and correlation to the University’s Five Pillars of Nursing (professionalism, critical thinking, holism, communication, and caring) on the overall experience and correlate to The Essentials of Doctoral Education for Advanced Nursing Practice.-

Note: Each week, your reflection should be a 1- to 2-page summary of the week’s activities. This should result in about 5 pages of the overall reflection summary for week 10.
Rubric:
• Be specific with the activities that were included in clinical hours. This should be more detailed than what is listed in Core. Include specifically what was done during the clinical activity time. Include more than literature review, or worked on IRB, or patient care hours, or met with preceptor. What did you find during the literature review, what specifically did you work on for IRB, what was discussed or completed with the preceptor, what did you do during patient care hours? Include any workshops, business or professional meetings. May include agendas or brochures if they are available. These are some examples of how to be specific. Your grade will be affected if you are not including specific activities, in addition to meeting the criteria listed in the grading rubric and the guidelines under week 5 project.

• Clearly reflected on the overall experience on the basis of each of the University’s Five Pillars of Nursing
• Used correct spelling, grammar, and professional vocabulary. Cited all sources using APA format. No errors. (articles must be within 5 years)

Note: Basically, this is what I have been working on for the DNP project:
Title:“Structured educational intervention with culturally tailored Human Immunodeficiency Virus (HIV) education to improve outcomes in African American male patients”
Revised IRB application revised and approved
• University Self-Consent form- Revised and approved
• Recruitment flyer/email- Revised
• All Education materials including editing powerpoint to include cultural teaching- revised
• All Pre and Post-test and demographic form- revised
• Letter of approval from IRB stating I have been approved on 08/01/2025. Now that I have been approved I have received clearance to start implementing my project.

Note: For weeks 8,9, and 10- hours were completed as project was turned in to IRB for approval. Approval was received in weeks 8 for minor corrections. Corrections were made with the assistance from preceptor and faculty. IRB application was resubmitted for official approval. Official approval received in week 9. Week 10, I am working on combining all assignments completed throughout the weeks such as methods, data collection tools in order to implement project.
VI. Interprofessional Collaboration for Improving Patient and Population Health Outcomes Emails with committee In week 6, I Continued edits to DNP project for the application and supportive documents. Multiple revisions made applying comments from Dr. Jones and revisions added to the IRB table in a different color. Discussed project with my preceptor to adjust the time of the project according to the comments received by Dr. Jones. Inquiry email to use PozQoL sent to La Trobe University explaining my intended use. This email was sent to Dr.Jones as well as my professor.

VIII. Advanced Nursing Practice Emails with committee In week 6, Spoke with my professor to make additional edits sent to Dr.Jones. Constant dialogue to determine if screening tool needed permission to be approved and if permission was granted. Approval email received from La Trobe University to use tool that is free online. Multiple revisions completed to PowerPoint to make smaller in size in order to fit into compressed zip file. Multiple revisions made to compress zip file to 5MB. Received signature from Dr.Jones on application and my professor to be submitted to IRB for approval.

VIII. Advanced Nursing Practice Other (comment below) Substantial Methods Project In week 6 we discussed, Methods project developed for DNP project. This methods included an overview of the approach/Design, site and participant information, and methods of my project. This section was a step by step of how I plan to implement my project and was clear direct and to the point.
VI. Interprofessional Collaboration for Improving Patient and Population Health Outcomes Other (comment below) Project Methodology and statistical development In week 7, We reviewed the methodology and statistical development for my project. In reviewing statistical tests, I was walked through SPSS for various tests such as paired t-tests, one way ANOVA, chi-squared tests, and correlation analysis. We developed a plan for statistical analysis and what potential tests will be used to be support my research data. Also, the rationale for why the tests are going to be applicable and applying SPSS to calculate the descriptive statistics. This helped to prepare for the part 2 of my methods project. The instruments that I’m using and its validity was discussed. I will be utilizing the pozqol questionnaire which is specifically for HIV positive people and it assesses overall well being. This will be implemented at the beginning for a pre test and at the end for a post test to measure if the quality of well being change. The DNP manuscript was also reviewed to better equip me to piece things together for my final manuscript. We are working together to break things into pieces for my project

In week 8, I submitted to the IRB and awaited approval from the IRB.
I received a response from IRB stating I was contingently approved and had to complete a IRB table of the changes and resubmit a new application and folder.
In week 9, I received my long awaited IRB approval for my quality improvement project on Structured Educational Intervention with Culturally Tailored HIV Education for African American Male Patients.
• In week 10, I discussed with my preceptor the plan for implementing the project and reviewed the objectives in relation to my project.
