One of the most significant objectives that all healthcare providers would want to record is a positive change. Planning the community on any intervention to solve the identified problem is important in meeting the desired outcome. Nurses have a core role in ensuring that individuals and populations have positive healthcare outcomes and improved patient outcomes (Mathieson et al., 2019). The implication is that nurses have to engage in activities designed to foster positive patient outcomes and enhance organizational processes. This implies that nurses have to appropriately identify the problems and plan to use the most effective intervention for better outcomes. Therefore, the purpose of this assignment is to draft a change proposal to justify a recommendation for improving the health of the community and establish the change as a priority for the next budget cycle.
Benefits of Proposed Health Care System Changes and its Implications for a Community.
Over the years, new approaches have been introduced in the healthcare system to heighten disease prevention, better treatment, and adopt effective management of diseases. One of the most important approaches that have shaped healthcare recently is adopting modern technology (Mathieson et al., 2019). It has played a huge role in enhancing diabetic treatment. In the assessment of the Jordan community in Minnesota, diabetes has been discussed as the major problem affecting a wide range of patients in the healthcare system.
The technological strategy will be one of the most effective measures that can aid in reducing the number of diabetic cases in the community. The change would require all the nurses and the patient to adapt to the effect of the technological culture that will be adopted to solve the diabetic problem among the population (Fernandez et al., 2019). This will ensure that all patients have an equal chance to interact with the modern technology used to manage diabetes in the community.
Another benefit of the proposed healthcare changes is the reduced cost of treatment. The diabetic patient spends more funds on managing the disease and visiting the clinics more often for an injection which would be important in managing the disease for a few days. However, with the new technology, patients would have an automated system to control their insulin levels without visiting healthcare organizations. Diabetes is a condition where the body is incapable of producing enough insulin as required or fails to utilize the produced insulin as appropriate (Fernandez et al., 2019). Poorly controlled diabetes is one of the most chronic conditions. The identified population health problem relevant to the practice is poorly controlled diabetes among most patients receiving diabetes care. Currently, patients cater for the cost, such as insulin pumps provided by other organizations. If the clinic could procure these tools at a lower price and introduce them to diabetic patients after training on their use would improve the safety of diabetic patients (Fernandez et al., 2019). The current finances at the clinic do not allow it to apply better interventions that can significantly reduce the severity of diabetes in the increasing number of patients.

Introduction
To improve health care outcomes within a community, leaders must understand the needs of specific populations. In addition, it is important that leaders are able to gather and present persuasive evidence to decision makers to obtain the necessary funding and establish these needs as priorities for budget and resource planning. Thus, proposed changes should support improved outcomes and align with the current or needed health care policy.
This assessment provides an opportunity to propose a community health care change that is based on the community health assessment you completed in Assessment 2.

They have also asked you to submit your recommendation in a formal change proposal. You are confident that you can present a strong case for change, including a financial plan, that will ensure the success of this initiative.
