Preparation
Before completing this activity, review the following exhibits in Chapter 2 of your textbook:
Exhibit 2-1: The Nested Environments of a Health Care Organization: Illustrates how external pressures can come from the general environment, the health care system, and the service area.
Exhibit 2-3: Determinants of a Service Area: Lists how consumer factors and market/provider factors influence patient behavior.
Exhibit 2-4: Types of Change: Visualizes how changes may be incremental, transitional, transformational, or disruptive.
As you review these exhibits, focus on how each helps you interpret what is happening in a healthcare setting and understand which issues are most significant.
Scenario
You are the newly hired Director of Strategy at Valley Regional Medical Center, and within your first two weeks on the job, the hospital is already under pressure. A local news station aired a story highlighting long emergency department wait times at your hospital, featuring interviews with frustrated patients and families. The story quickly spread across the community, and since then, patient complaints have increased, staff reports patients are arriving angry and distrustful, and scheduled appointments have declined by 7%. At the same time, a competing health system has opened a new outpatient center just eight miles away, offering same-day appointments, extended evening hours, and aggressive marketing focused on “fast, convenient care.”
Internally, the situation is becoming more complicated. The state has just implemented new public reporting requirements for wait times and readmission rates, meaning your performance will soon be visible to the public. On top of that, Medicare has announced upcoming reimbursement cuts for one of your hospital’s highest-volume service lines, creating concern about future revenue.
Leadership is divided, and tensions are rising. Some executives believe the situation is temporary and driven by negative media attention, while others argue that patient expectations and market competition are permanently shifting. Clinical leaders are focused on care quality, finance is focused on declining revenue, and operations leaders are dealing with complaints and throughput challenges. At your first executive meeting, the CEO turns to you and says, “We cannot fix everything at once. I need to know what is really going on here and what we should be paying attention to first before this situation gets worse.”

Assess the speed of development (evolutionary or revolutionary) of at least two major external pressures in the scenario and their impact on the hospital’s current strategy (reinforcing or counter to).
Identify the issue that appears most significant based on your analysis and support your choice with at least two pieces of evidence from the scenario.
