In order to meet the module learning outcomes and pass this assessment, you are required to:
Reflect on a chosen model of partnership working in the context of health and social care. This should be presented as a:
written reflective essay of 1000 words (+/- 10%), excluding references.
Your essay must address the following sections:

- Briefly introduce the concept of partnership working and its relevance in health and social care.
- Identify and briefly describe your chosen model of partnership working (e.g., integrated working, interprofessional collaboration). State why you selected this model for reflection.
2. Theoretical Underpinnings and Key Components (Approximately 350 words)
· Explore the theoretical underpinnings of your chosen model of partnership working. Discuss its core principles and values (e.g., shared goals, respect, trust, open communication, accountability).
· Describe the key components or features of this model and how they are intended to facilitate collaboration.

3. Benefits for Service Users and Practitioners (Approximately 250 words)
· Based on your critical reflection and drawing on relevant literature and real-world examples, discuss the potential benefits of applying this model for both service users and practitioners in health and social care.
