An incident occurred at a food manufacturing plant when a worker’s hand got caught in one of the food processing equipment. The trip switch of the equipment immediately stopped the processes, and this enabled the other workers to assist with getting the worker trap hand out. The injured worker had to go through an urgent surgery and was hospitalised for 3 weeks and given 2 months of medical leave.

Based on your understanding of the Workplace Safety and Health Act and continued safety operations at the manufacturing plant, describe the employer duties, and follow up actions from this accident.

(b) Examine and describe TEN (10) requirements a responsible person must ensure before allowing anyone into any confined space.

You are appointed as project manager at a construction site and is very concerned with site safety after several near mishaps. As a certified safety officer, you decided to carry out a risk assessment of the construction site. A risk assessment team comprising of a site supervisor, a contractor site foreman, an experience construction worker, and contractor safety officer is formed. You instructed your risk assessment team to submit the risk assessment within 4 weeks and must include the following:

(a) Identify and elaborate the work activity and location for understanding on at least TWO (2) hazards which potentially can cause serious accidents.

(b) Examine the potential ill health/accident which the hazards can cause.

(c) Recommend and prepare risk control measures which can be implemented to reduce the hazards.

(d) Appraise another risk control measure which can eliminate the hazards.

(e) Prepare a complete Risk Assessment Control Form report to be submitted to the project manager for his approval for implementation of the risk controls recommended by the team.
