Module: Care Support – 5N0758
Guidelines:
Learners are required to submit a 5 day personal journal or 1500-2000 words that includes a description of the learner’s relationships with people they are in contact with on a day-to-day basis in their vocational areas including their colleagues, the individuals in their organisations and their families and other personnel.
The report should include:
- A description of specific events and interactions with members of the multidisciplinary team;
- A review of the interaction in terms of respecting diversity and individuality of co workers and clients;
- A considered daily reflection on conflicts and challenges that arose and strategies to deal with them effectively
- The importance of all forms of communication in client care;
- An outline of what the candidate would do differently in any future interaction to support the needs of the client;
- A detailed reflection on own personal and professional performance, strengths and weaknesses and the development of strategies to improve in the future.
The learner record should include at least five individual experiences where the candidate has provided care for a client and/or their family.
Marking Criteria
