Ms. Ewing is a 23-year-old female who comes to the clinic to be evaluated for her asthma. She reports wheezing and coughing 1 or 2 days each week and awakening from sleep three or four times each month with asthma symptoms. The cough is nonproductive. Her forced expiratory volume in 1 second (FEV1) is 78% of the predicted value. Her current medication regimen is an albuterol metered-dose inhaler, 2 puffs every 4 hours as needed. She admits she has been using the inhaler several times daily for the past week or so. She denies any food or medication allergies. She is taking no new medications (including OTC). She denies smoking.
VS: T 98.8o, HR 88, RR 18, BP 128/68. LMP started last Tuesday. Her room air oxygen saturation is 97%. She is AAOx3. Her heart rate is regular without murmur or gallop. She does have bilateral expiratory wheezing, but the respiratory exam is causing her to cough and become anxious. The cough causes severe bronchospasms so you really cannot fully assess her lungs.
