Examine a population of people that may be considered vulnerable and/or underserved due to disparities related to healthcare that you may have experience with or provide services to. Vulnerable or disadvantaged individuals are people that are economically disadvantaged, the elderly, racial and ethnic minorities, the unemployed, uninsured and under-insured, children of low-income families, LGBTQ+ and gender non-conforming, people with HIV, people with severe mental and behavioral health disorders, the homeless, refugees, and many others. These populations of individuals are predisposed to poor health, chronic disease, disability, and early mortality.

Answer the following questions related to this case study. Support your answers with rationale. Cite at least 5 current, peer-reviewed, scholarly sources in your answers and include a reference list.

  1. Identify your chosen vulnerable population and describe the group. How is this population underserved? (Support your answer with statistical data that supports why they are underserved.)
  2. What are Social Determinants of Health (SDOH)? Define this term and discuss the five key areas that are addressed. How are SDOH applicable to your chosen vulnerable population?
  3. What are the national objectives for Healthy People 2030? How are the national objectives guided by data and the SDOH?
  4. What is the current health status of the vulnerable population? What is currently being done for this population and this health issue? Is it adequate? What needs are being addressed? What needs are currently still unmet? Considering the vulnerable population identified in one, what actions as a CNS can you can take to improve the SDOH for these individuals? What Healthy People 2030 objective(s) can you impact as well?
  5. Finally, how can you incorporate technology into practice in order to address the health needs of the vulnerable population and improve outcomes?
