In a recent experience on the Medical Intensive Care Unit (MiCU), a patient with severe sepsis was admitted, and the treatment plan was formulated without incorporating the patient’s preferences, social determinants of health, and values. The patient, Mr. X, was an elderly individual with limited English proficiency and no family present to advocate for him. Despite attempts by the healthcare team to explain the treatment options, Mr. X appeared confused and overwhelmed, and his cultural background and beliefs were not adequately considered in the decision-making process.

 

As a result, Mr. X’s treatment plan did not align with his preferences and values. He received aggressive interventions without fully understanding the potential risks and benefits, leading to increased anxiety and distress. Moreover, his lack of social support and access to resources, such as transportation and financial assistance, further exacerbated his situation and impacted the outcome of his treatment.

 

Incorporating patient preferences, social determinants of health, and values into the treatment plan could have significantly impacted the trajectory of Mr. X’s situation. For instance, by utilizing a decision aid focused on sepsis management, healthcare providers could have facilitated shared decision-making, ensuring Mr. X’s understanding of the treatment options and considering his cultural beliefs and language barriers. Additionally, addressing social determinants of health, such as connecting Mr. X with community resources for ongoing support post-discharge, could have improved his overall well-being and outcomes.

 

The value of the patient decision aid lies in its ability to empower patients to make informed decisions about their care. In Mr. X’s case, a decision aid tailored to sepsis management could have provided educational materials in multiple languages, visual aids, and simplified explanations of treatment options. Moreover, the decision aid inventory from the Ottawa Hospital Research Institute can be utilized in my professional practice by incorporating it into patient education sessions and discussions with interdisciplinary team members to ensure comprehensive and patient-centered care delivery. Additionally, I can utilize it in my personal life to make informed healthcare decisions for myself and my loved ones.
