SOCW 4304- –Logic Model of your Field Placement/Agency Rubric For this paper, you are required to discuss the following concepts and ideas below as it relates to  your field agency placement. Remember that a logic model is a tool that an organization or  agency uses to either evaluate or measure how successful an agency is in implementing what it sets out to accomplish. A logic models also measures outcomes of an agency.

By this point, most of you have already asked your field instructor what the logic model that your agency utilizes. This paper is to help you practice Competency # 4 which addresses  practice informed research and research informed practice.

In a 4 to 5 page paper, you are required to write about the logic model that is used by your  agency, such as specific elements such as resources and budgeting. Your agency may use another name for its logic model or a tool to measure outcomes and effectiveness.

Your paper must address the following components:

What is the Logic Model used at your agency or tool used to measure the  agency’s effectiveness?

Discuss what a block frame planning tool is and how it relates to your agency.

Discuss the relationship between program goal and program objectives, resources,  activities and outputs in your agency.

You should be able to operationalize outputs as an annual measure of activities.

Discuss what an implementation assessment is in your agency.

Identify case record review as a form of qualitative research that is single subject  design as it relates to your agency.

You should know that collecting sociodemographic data on program service  recipients is descriptive, practice research. How is this information used at your  agency? Or not? Please discuss.
