TASK DESCRIPTION
Your task is to produce an information booklet of long-term health conditions and the management of these conditions to health and care service users or professionals in the UK. Your booklet should adopt language and concepts suitable for the chosen audience and be referenced throughout. You are free to choose the specific long-term condition you will focus on for the
information booklet. Options will be discussed and decided through the taught content in class. Submission is in week 6. A booklet template guide will be provided and you should use this to guide you.
This information booklet should include the following:
An explanation and evaluation of the focused long-term condition including the definition, burden/prevalence of the long-term condition (both worldwide and UK statistics) and the risk factors/aetiology of the health condition.
Module Learning Outcomes
• Critically evaluate the physical, psychological, and social impact on individuals and families living with a long-term condition(s) and its treatment.
• Critically analyse the contribution of empirical evidence and local, national, and international guidelines to informing the delivery of ‘whole person’ care and self- management support for individuals with long term conditions.
