Using a problem that you have witnessed at your placement site or that you foresee in a program you are creating, develop a quality improvement plan for improving work processes. Your quality improvement plan should include a description of the problem, delineate what you will measure, the frequency of measurement, and any supplemental measurements needed to inform your QI team as you attempt to improve that process (Narrative or paper format for this section). In addition, choose two of your program goals and develop tools for evaluation using a 360 evaluation format. My expectation for this is that you will choose at least three communities of interest (client, community partners, employees, the board of directors, etc.) and develop a survey or questionnaire they can answer that will give you information about the effectiveness of the program and based on the goal you are evaluating. If your goal is too broad, choose one of the objectives from two different goals. The questions will be similar for the three surveys, but you may need to slightly change the focus. This is a total of 6 surveys (minimum of 10 questions each survey). Identify a timeline for distribution of the surveys, how often you will distribute the surveys, and how you will analyze the data. Your assignment will include the following elements: 3 pages (this is double-spaced, but if you choose to do single-spaced it would be about 1-1.5 pages) describing your quality improvement plan for a problem dealing with workflow or work processes. Your workflow quality assurance plan will include: Description of the problem What you are measuring and why you are measuring it Frequency of measurement Supplemental measurements needed to improve this workflow problem 360 evaluation of two program goals and/or two objectives (depending on which makes the most sense to evaluate). Your 360 evaluation will include A brief description of each goal/objective and the 360 evaluation process A brief description of the communities of interest you are going to survey and how getting information from them will help you determine if you are achieving your goal or if the treatment protocol, research, evidence-based treatment, etc. is effective and doing what it is supposed to be doing 3 surveys for each goal/objective with a minimum of 10 questions each, totaling 6 surveys (the surveys will be similar, but you need to make sure it makes sense for the community of interest you will be giving the survey to) For example, if you are going to build a relationship with a referral source as one objective you may survey the clients who are receiving services as a result of this referral, the individual(s) offering the referral, and the administrator or director who oversees the referral process. If your second goal for example is to improve clinical skill of pre-licensed clinicians, you may survey the client’s served by pre-licensed clinicians, the clinicians, and their supervisors. Conclusion/Summary of your quality assurance plan The format of this assignment should be one that you would present to a Board of Directors or communities of interest and not APA formatted. You may need to use charts, graphics, pictures, etc. to make your point and achieve an aesthetically pleasing document for those who will be reviewing it (Program directors, the Board of Directors, and potential outside communities of interest).
