DeNisco (2023) describes the traditional highly private insurance delivery system in America as fragmented and cumbersome. Because of the lack of standardization and governmental oversight in the American system, there are problems of duplication, overlap, inconsistency, and waste (DeNisco, 2023). The system is often described as inefficient and not cost-effective.

Currently, there is a major evolving shift towards the development Managed Care Organizations (MCOs) in an effort to provide value-based health care in America.  These MCOs coordinate care more efficiently by integrating specialty group practices into larger groups and alliances.  One example of this is when multi-specialty groups and practices join together to form an organization where there is common administrative oversight and the sharing of resources (DeNisco, 2023).  This integrated delivery format allows for smoother delivery of care, a reduction of overlap, and better control of costs.  One example of an MCO is Kaiser Permanente and the Mayo Clinic (DeNisco, 2023).

The introduction of the NP role into MCOs has contributed significantly to decreasing costs while maintaining high-quality delivery of care within an integrated network of providers (DeNisco 2023).    The NP can decrease costs within these MCOs by providing preventative care and managing chronic conditions for patients.  Because of this, the APRN role is rapidly expanding as Primary Physicians are recognizing the significant role that APRNs can play in decreasing costs within their organizations.  It is exciting that as a result, we now see a significant increase in roles and specialties in which NPs can function because of the development of these MCOs.
