Reviewing the Literature

Summary of Clinical Issue (200-250 words):
PICO Question: Can a nurse-led, transitional-care program, which uses a nurse-led, patient-focused, standardized education and early follow-up via telephone or telehealth, reduce 30-day hospital readmissions in adults with heart failure discharged from an acute-care hospital compared to usual post-discharge care? The secondary question is: Does nurse-led transitional care help adults to make more self-care and symptom-management behaviors after a heart failure hospitalization than usual care?
Part 1: Literature Evaluation Table

Part 2: Analysis of Literature
In 750-1,000 words, write a review of the literature. This section should comprehensively summarize previous research related to your topic and demonstrate support for your PICO question.
Remember, you are building an argument to for your proposed evidence-based project; this is not simply an article review.
Appraise each article and answer the following questions for each (one to two sentences is sufficient to answer each question):
1. Summarize the purpose of the article or research.
2. Describe how the article or research relates to the health care problem or issue you selected.
3. Explain how the evidence in the article or research supports your proposed intervention.
